I see it's very common for people mentioning that embassy's ask for Additional documents.

I'm just wondering what these additional documents are? I know everyones case is different so I guess just examples would be great to know.

Is it something that can be easily avoided if you ensure that that checklist is complete and you have a fair amount of evidence?

In the case of Kingston, additional documents requests can take the form of originals for birth certificates and marriage certificates. Additionally, they can ask for proof of continued contact between sponsor and applicant, especially if it's been awhile since the application was received.

Other embassies can send region specific forms, not sure what those would be, though.

for Middle East they ask a lot of things in general: yes continuous proof of relationship, they ask extra details abt the sponsor residency in canada (updated proofs that the sponsor is in canada, details abt the sponsor work, they even have a specific form called residency questionnaire to fill in), they could also ask official immigration document with the entry/exit stamps from the country u live (this is specific for the Gulf) and before they issue the visa they could ask work visa cancelation, proof of resigning from ur job for both the applicant and the sponsor if applicable

all these things r not part of any checklist and they don't ask from all people, not sure if it's random or they have some criterium...in general the best is to follow ur own embassy, manilla thread and u'll know their way
