Additional documents request-Help

I got today this message:

This is in reference to your application for permanent residence in Canada. In order for us to continue the

review of your application, we require the following documents:

1) My UCI number -and my name

2) Release of Information IMM5475: A signed and completed Authority to Release Personal

Information to a Designated Individual form (IMM5475) bearing original signatures

I didnt understand what they want me to upload in section 1 ?

You can write you name and UCI number in a letter of explanation along with this additional document request printed out. In additional, you can scan your work permit too.

Section 1 is the default email.. it is very confusing actually..

They did the same thing to me. asking my UCI#. I was confused as hell as well that time.

They just asked you the 2nd item actually

in my case it was as follows:




We have begun processing your application for permanent residence, and in order to make a decision we require the following information/documents:


 xxxx UCI number- my name:


 Additional Family Information IMM 5406: A separate and newly completed Additional Family Information form (IMM 5406) bearing an original signature. Please make sure to include information about parents.


please advise what to share for the first point
