Addition of my daughter to my family

Dear Members,

My application for FSWP is being processed by Warsaw-Poland office
Recently I informed them about my new born 2nd daughter and they are asking me to send some documents.
I got few doubts and since they are not replying I thought this is the best venue to get clarified.

1) What is the mailing address I shall send the documents? Is it Canada or Poland? Please provide complete mailing address.

2) The copy of Birth certificate shall be notarized or not?

You need to send all the requested docs by courier to
Canadian Embassy
Immigration Section
ul.Piekna 2/8
00-482 Warsaw, Poland


Also include the newborn passport copy, birth certificate, fees and all relevant forms.

make sure to include the Generic form updated with all the family names.
