About the Supplementary Information Form issued from Islamabad Office

Can anybody help me out in filling the Supplementary Information Form issued from Islamabad Office. I have following two questions:

Q1- What is the total number of passports issued to you including your current valid passport?

I have two passports one is valid and the other one is invalid which has some incorrect spellings that’s why the immigration office stamped it “invalid”. Do I have to mention this invalid passport as well.


Q2- Did you serve in any militia, army, defence or police until (including obligatory national service, reserve or volunteer units)? If no, please explain:

I never served in the army so my answer is No but what should I write to explain?

I would. Just explain why it is now invalid.

Is some kind of military service obligatory in Pakistan? If yes, but you didn't do this military service, explain why.

Thanks, canadianwoman. In Pakistan there is no obligation to complete military training therefore; I am confused what to explain.

If you read the instruction it says if not then write Not Applicable so just write Not Applicable
regarding your previous passport if u had any traveling on it then you have to mention it.
If there was no travel and it was expired still u have to mention it.

Though I did not get the AOR as yet but I took your suggestion and filling out the form in advance. Its been three month I did not get any response from Islamabad office.

Don't worry you should be getting it soon just keep all the documents ready in advance like supplementary form, recent proof of communication, wedding and reception photos if u did not submit them already and original Nikkah Nama if was not submitted earlier.
