About the dates that a Visitor status was obtained/effective" and expired, I'm really confused. Plea

Question 9 b) During your eligibility period did you have Temporary Resident or Protected Person status in Canada before becoming a permanent resident?

If yes, provide details in the chart below.

I visited Canada three times with a Visitor visa before I became a permanent resident. So I need to fill the form.

The details of three visits are:
From June to July 2011, on a single-entry visitor visa.
From December 2011 to January 2012, on a multiple-entry visitor visa.
From June to November 2012, on a multiple-entry visitor visa.

My questions are :

1)Since the question only asks about my status during the eligibility period, which means October 2012 to October 2017. So I guess I only need to list my third visit (June to November 2012). Is that correct?

2) Date your status was obtained/effective (YYYY-MM-DD). Should I write the date I was issued a visitor visa or the date I entered Canada with a visitor visa?

3) Date your status expired (YYYY-MM-DD). Should I write the date my visitor visa expires or was invalidated or the date I left Canada as a visitor?

Thank you very much!!!

1 - Oct 2012 to Nov 2012 - the calculator form wont even let u add anything before Oct 2012 ( this whole things revolved around the edibility period )

Edit: In Q. 9 - Eligibility - Oct 2012-Nov 2012 and under Status you will write June 2012-Nov 2012

2 - Yes, date when you entered

3 - Date you left, not the expiry of visa since the the half time is only for time spent so visitor status i.e, 1 month
