A question about Consent Form for "GSCM NOTES''

Hello everyone,

we applied for the GSCM NOTES by submitting the consent form, and they sent us an email that says:

Please resubmit the consent form with section 3 duly completed to the e-mail address indicated below.

so that is the section where it says : Your designated Individual's full name

whom we gonna put there? my name as I am the applicant? or my wife's name as she is the sponsor? or what exactly?


we don't understand why they asked us now to fill up the section 3, because we did already applied for notes about 4 months ago and we didn't fill up the section #3 and still we did receive the notes.

All they want is that the applicant ( you ) is giving consent to whoever ( sponsor for eg., ) to order these notes. So yes, you can put wife's name

Thank you so much for the quick reply, do we need to submit a new form or we can just use the same form that we submitted and just write my wife's name just by pen by hand?

submit new form..my friend had the same problem..
and designated individual is the one who is applying for GCMS notes on your behalf..

We submitted a notarized IMM5475 "authority to release information to a designated individual" with my husbands family class sponsorship application, do we still need to submit an IMM5744? If so, what does an IMM5475 do? Does anyone know?

Hello Friends,

I need one help for the consent form

So the primary applicant is my husband, and i am currently in Canada and he is in India ,I used GSCM buddy , so there is a designated representative on the form, primary applicant is my husband who is in India, but in the form there is my name as a relative applicant on the option third , do they need my details.

Could anyone can help me for the same.
