A few questions regarding Option C printout

I called the Canada Revenue Agency for the Option C printout, then they sent me a paper called Income Tax Return Information-Regular. Is this actually the Option C printout?

One more thing, .. today is Feb 18, 2013 and I haven't filed my income tax return for the year 2013 yet, so my last Notice of Assessment for the most recent taxation year is 2012 right?
