a few questions about BOWP

hi everyone, I'm preparing my BOWP application and got two questions about IMM5710E form.

On page 1, Question 3, should I select 2 or 3 below?
"I am applying for one or more of the following:
- A work permit with the same employer
- An initial work permit or a work permit with a new employer
- Restoration of your status as a worker
- Temporary Resident Permit

On page 2, What kind of "Document Number" should I provide?
"If applicable, provide the document number of the most recent Visitor Record, Study Permit, Work Permit or Temporary Resident Permit issue to you."

I chose a work permit with a new employer.
Client id is the document number that you will see on your all work permits/study permits until now.

2) it is the file No starting with "U" in your wp.

Can you specify your question? which one are you referring to put N/A? you mean the document No? this No is on all wp, not sure why you would put N/A?

Hi!
I am trying to apply for BOWP online but it's asking me to upload Proof of Medical Exam , what should I upload as I dont have any report from the doctor (I had my medicals for CEC, back in August) also how did you take your picture I mean what did you do for frame size and pixel requirements.

Please check if you have selected any option that you will be working on health care?. Also if you were in other countries more than 6 month in last 1 year, you may need to do medicals. There are number of free software available in internet to adjust your scanned photo to the required size.

In IMM 5710 - leave fields blank if they don't apply to you.
