5669e: Do I have to fill Questions 7 to 10 when my answers to Q6 are NO

I'm filling out Form 5669e - Schedule A Background / Declaration. My Answer to Question 6 is NO for all options.
and in the end it says I should only provide details only if my answer to anyone is YES.
SO does that mean I have to leave Questions 7 to 10?
