3 simply questions, please.

I'm reviewing my application forms and having the following 3questions:

1). in IMM5406 - additional family information, it says"complete all names in English and in your native language (for example, arabic, cyrillic, chinese, chinese commercial/telegraphic code .....)", my passport shows my English name as XIAOHONG LI. but in Chinese(native language), it should printed as "LI XIAO HONG". So i just wondered should i just list my passport name which is in English? or just what people usually call me in Chinese? or both of them, if so how do I present it?

2).Generic Application Form for Canada [IMM 0008], Question 10: Current country of residence. i was an international student from 2008.09-2011.08, and i'm now in PGWP since 2011.08. When I filled this section out, my status should be: Worker, but for the date from, should i fill as 2008.09 when i started living in Canada? or 2011.08 when i started being a worker here?

3) in some forms of additional paper, i use the number format as "From: 2011.08" "To:2012.08", anyone knows if "2011.08" is the appropriate way to present 2011 August?

Let me start with your second question...

as you entered Canada as an Intl Student, you need to mention as "Student" from the drop down list. The "To" date for this status should be as of the day you are filling / can be future date till your status expires.

Your correct status split as student and PGWP can be shown in "Schedule 8 - Canadian Economic Class" application.

Coming to your IMM5406 ... name in Native language .... You need to mention your name in as it is printed on Passport. i.e as "XIAOHONG LI" ( I think it is 李晓红, got it using a translating tool)

3rd question: In most of the cases, the format of the date is already mentioned in the application and you need to fill in as mentioned. Where there is no format mentioned, you need go with only numerics. 2011/08 can be mentioned as 2011- Aug in order to avoid any confusion.
