Guys please advise.

I have only worked for 1 company for period of October 2013 till April 2018

October 2013 - June 2015 - no unpaid leaves or extended sick leaves, just few days off on vacation visiting home
June 2015 - August 2015 - Medical leave - paid leave.
September 2015 till October 2016 - no unpaid leaves or extended sick leaves, just few days off on vacation visiting home
November 2016 - time off for relocation - paid leave
December 2016 till November 2017 - no unpaid leaves or extended sick leaves, just few days off on vacation
December 2017 - Jan 15 2018 - Unpaid leave
Jan 16 2018 - April 16, 2018 - no unpaid leaves or extended sick leaves, just few days off on vacation

Does anything from above constitute as break in employment?

How should I report the periods of employment in work history if there is a break in employment?

Your long medical leave in 2015 was paid. Your relocation period in 2016 was also paid. So I don't think it can be counted as a break. You can show this as continuous 4.5 yrs of work experience.

What about December 2017 - Jan 15, 2018? That was unpaid leave. Should I state Oct 2013 till November 2017 and then Jan 2018 till April 2018 as two separate entries in work history? or a continuous Oct 2013 till April 2018 is fine as long as sabbatical is mentioned in reference letter and personal history section?

That was unpaid but by then you have already crossed the 3 yrs employment time period. You get maximum points for proving 3 yrs of work experience. So even though it was unpaid, it was short and can't be counted as a break and you will still get all the points you need.

Agreed. But the reference letter will be for the entire period from Oct 2013 till April 2018 that includes the unpaid time off. Wouldn't it be a misrepresentation if sabbatical is not stated in my work history, personal history and employer reference letter?

During the sabbatical if you travelled to another country then you'll have to mention that in travel history.

But in work history you will still show that you worked for 4.5 yrs because you dint change jobs and you were still with the same company. And because your unpaid leave is in the fourth yr it doesn't affect your points which you'll get only on the basis of your first 3 yrs of employment. If you had a long unpaid break anytime in the first 3 yrs then you would have to mention it.

Thank you for your prompt responses. The sabbatical was actually in my 5th year and I believe my employer will mention the same in the reference letter. So to avoid conflict, shall I report my work history as following?
Oct 2013 till November 2017 and then Jan 2018 till April 2018

then I will mention December 2017 till Jan 2018 as break/holiday in my personal activity so that it matches with employer reference letter. Will that be a problem?

You don't need to break up your work experience. You need to do that if you want to show two different jobs. Since your employer is going to mention it on the letter then that's perfect. You don't even have to explain it.

In your personal history you only have to enter long term addresses. So even if you travelled anywhere for 1 month or so during your sabbatical there's no need to include that in personal history.

If you travelled anywhere in the last 10 yrs include it in travel history along with scans of stamped pages of your passport. This doesn't have to match with work or personal history.

NO, it will not be. Just get a reference letter with From to TILL now. that's it.
