Your wife can take your baby out of the country without even being asked a thing about it. I am speaking from experience, my child has Canadian citizenship, because of her father. I have a passport for my country, and she has her Canadain PP. We have different last names on our PPs too...

If you are doing an "outside" application you need to order the region specific forms from CIC, these will be delivered to you by mail. Not true! I did an outside application and used the forms that I got of the CIC website. Also I was never sent any additional forms to fill (Region Specific)...
