Yes, if it says that under review of additional documents then it has been uploaded successfully. Good job. :) Also if your application status went from your action is required back to submitted then it is also an indicator that you have provided the adr successfully.

Whichever one is closer but there are specific panel physicians you have to use for the medical so google them on the CIC website and it will give contact information to make an appointment with. I don’t know how much it is for the medical unfortunately but hope this is helpful.

No this is not pre-arrival. Under review status just means they are working on your file over time. Pre arrival will show as a new message on the online application as an attachment if you get it or at least that is how my wife’s happened.
