Seems like you already living outside of Quebec for a while, isn't that enough for IRCC ? why they still asking for proofing of intent ? I have a similar situation like you, by any chance did you check with IRCC status of eligibility, such as met, review required??

the medical part is the most easiest part so the officer may chose to update in the later stage, it didn't mean anything.

From what I understand if you are using cic portal upload the requirement, it takes 6 month you will get the result, no ideal where it states 18 months, maybe you mean by paper filing?
