I've submitted my phone bills and bank account statements. I didn't submit all of them though, only four statements per year. I submitted phone bills for January, April, July, October and bank statement for February, May, August, November. And I did it for both years.

Don't know if mandatory, but I did submit my transcripts when I applied. You want your application to be as complete as possible to avoid delays. And yes, you can submit your eMedical forms with your application.
