Thanks @canuck_in_uk. Got Dr Reznek's info from the same Panel Physician's page. It just has his phone number and it seems to be outdated. May be I should visit his office in person to figure it out.

Can someone please share Dr Reznek's contact. Tried calling his office and cannot reach him. Couldn't find website details too.
