Yes, I used that one, the latest one with the left footer "06-2019". I'm using the Adobe Acrobat Reader DC (19.012.20034). I also applied for visa last time with no problem but I don't know why I got stuck this time. Hope you can help me figure this out. Thank you!

Having the same problem with IMM5257e form for 2 days straight, I tried so many ways switching pc, switching browser, downloading a new form and filling in but still, didn't work at all. Send help, any advices is appreciated!
