yes its been 4 months since am in canada i dont have a proof thats the problem that was just a simple receipt and now id ont have it

AM USING THE right form :/ Have you had a medical exam performed by a CIC authorized panel physician (doctor) within the last 12 months? this is the question and if i select yes they ask for proof it is obvious that i had a medical test before coming to canada
