has anyone requested for providing additional family info , form 4506 to be filled?? If someone is legally separated then does he need to mention spouse details or simply mark as N/A?? How to submit the form in their office if someone is outland applicant? Kindly help or suggest

I h I got the update on my medical just today... and asked for biometrics. Any idea if background check depends on biometrics result as I an asked to finish within 30 days....

I have no other doc and how long do they allow time to send you additional supporting docs ? I am afraid to loose chance in ITA...

thanks for the suggestion. just to clarify that if spouse is not supportive can i sign and prepare affidavit by myself? Also, what if I apply from India? I can still use this format and sign by myself and notarized in India. will it work?
