Congrats to you and your wife as well! I applied without any dependants, so my ANNEX A only included my name and was only one page. All you need to do is print the ANNEX A, fill it and send it. Cheers!

You could always try, but at this point I would suggest that you order the GCMS

Hi, Only one update occurred since submission of the application. Please see all details here and here. I got 1 year. Valid until 08/17/2019. Which leads me to believe that they give 1 year from the moment the application was approved (i.e.: independent from current work permit expiry date)...
