When uploading the information from the doctor, the only thing I include is the print out sheet? This just had my information, the test required and my photo, no actual results The panel physician will upload the results through the e-medical system them self?

Is having a mortgage/loan on your account where you have your proof of funds seen as bad...the proof of funds are available but I also have a loan on the account. Or is it best to not have the funds held on the same institution where you have loans
