Nothing is easy but where there is a will then there is a way - study and it's not impossible. I'm a native English speaker and I didn't get the highest score so I know that it's not easy but that is in the OP control as they can do it multiple times. Also the highest on the IELTS is not required.

Your CRS score is subjective as you don't know the OP details. I just ran the calculator for a couple with no Canadian experience or Canadian education and the score comes up to 494. So as you can see the OP should run the calculator and see what score they comes in at. Depending on the score...

There are many pathways ... one of which is the direct federal EE pathway which does not require a study permit nor work permit unless the rules have recently changed.

For PR purposes, OP might need to provide a reason for the out of status (valid reason is subjective). If OP applies for a visa other than PR then the reason for overstay becomes critical for approval (needs to convince officer). Now we don't know the overstay period by the OP in the USA ...

Send an email to Ottawa requesting the courier address while explaining the problem that FedEx does not deliver to PO Boxes. I did the same and they sent me another PPR request with the physical address for courier delivery. They might take a few days to respond. All the best.

It’s up to the IO what they do. They can ask you to redo the medical or can choose to extend it. What rules govern who they are choosing to extend is unknown. Best of luck.
