If you have that, you need to enter that in Medicals completion proof. Mostly your "Information Sheet" will have that detail - so you dont need a separate section for that. I am using an old medical (valid for 5 years) so i included a LOE and in that i included the IME/UMI number.

I dont think the question is too literal but yes you can elaborate different addresses even if its the same city or country. If you know the dates, there is no harm in giving the addresses. Also, address on passport can be different so that is not something that is asked in the application...
