That is just a formality done by the doctor for small children. He will just check with his stethoscope and nothing else. You will have to get it done from an approved physician only, from where you are getting your medicals done.

WES sends a soft copy in your email account. You can also download a copy from your WES account. Once you have the ref no from the WES report you can enter the pool alongwith your IELTS score obviously.

You can get this without your spectacles without any issue. Also no need to explain also in my opinion. Even if you decide to explain about this you can mention in the LOE document that you would upload for all other explanations.
