The validation button should appear at the bottom of the form right below where you write out the full name of the applicant as their electronic signature. The form was recently updated, I think the new version should be "IMM5669 (02-2018). Ensure you're using the latest version.

Well this depends on the physician you go to do the medical...most should use the E-Medical system which they use to upload the result for you. They might also give you a medical information form and tell you to send it in to notify CIC that you have done the test. They will send in the result...

Have you tried using the application number, and the surname option? Also if your number of person in the application you submitted is one, try using two when linking the account.

No, they should also send your way an email with her own UCI. However, the application number should remain the same for both of you.
