I did the same. I submitted just the downloaded option c of my wife for the three year. I submitted it on 13th June. Status still the same. Application received only.

1. My advise is write NA. I did the same. So far no issues 2. If in canada > 6 months (like supervisa) write canadian address. If <6 months, write home address (india in your case).

Really I dont think its needed. But why take a chance. The email specifically mentions "original option c print". So i plan to send the original as well once received from CRA
