My postal code(CA address) was indicated in the 5406 but my wife's (PH address) did not include postal code, country was indicated however. Got AOR already :)

Open Adobe Reader first and then open the file from the File menu in Adobe Reader. You might be double clicking the document and somehow your browser is the one set to open PDF files by default. :)

Also had the same problem during the weekend, i had a mismatch with the Place of birth for the PA and as @posse said the number of people in the application should include the sponsor.
