    HI, You don't need to fill out Most recent entry date if it is same as original Entry date. Leave #3 section blank. it say Date & Place of your most recent entry to Canada (IF NOT SAME AS ORIGINAL ENTRY) You only need to fill # 3 if you have 2 different dates.
