Hi all! Do we need to submit something for medical? My understanding is doctor submits directly. It's odd because doctor sent us a eMedical Information Sheet but the test says incomplete. Not sure what to do with it... Thanks!

I did get it to work... but I can't recall what the trick was! it was back in may. I think it want you to check a box, that you don't want to check as it doesn't apply to you. I think I checked it, and then used white out.

I had the issue We started the form again and then got it working

had a similar issue but re-dowloaded form and completed it again and it worked

if you try to get form today there is a June version
