Replying to my own post. I have an official response now from Passport Canada that References can be in our outside USA or Canada. So, I'm not sure why an agent has rejected it with a handwritten note saying exactly the opposite. I will be filing an official complaint.

Yeh, there is not much information around for OCI holders. I'm going to submit mine as received, there is an issue date on the PCC and that is after my last visit to India on OCI. I'm interpreting the CIC requirement as fulfilled, since the issue date is after my last trip to India.

I got my PCC from the consulate. Apart from mentioning my current nationality, passport number and OCI number, they've also included details of my surrendered Indian passports. Furthermore, they've stamped my current passport. Wondering if this is good enough. /D
