I am not sure if I understood the question. But what I can say: I got my medical request in my Gckey account too. ( a message in mycic), and it didn't indicate my visa office. Mycic account wasn't updated directly and didn't show that I have done med exam immediately. I had to wait about 10...

Yep , flagpoling is definetely the easiest solution for most people. But for me i didn't want to take any chances with us customs (not from visa-waived countries, expired us visa, and my first name can tell my religion if you know what I mean haha).... So Ottawa was the best choice for me...

If you want to avoid going to the border you can set up an appointment at cic office near you, by calling them, but delays can be long i heard. For the province you just have to your intention to live in quebec. If they detect you wanna go somewhere else it can cause problems i suppose. But...
