Regarding question #4g showing place of birth . . . Instructions say to list the place of birth as shown on my passport (U.S.A.). U.S. passports show the state in which I was born. There is no "city/town" put on the passport. My question is do I put the state listed on my passport where it says...

Greetings . . . I received the same as most here . . . a request for a medical exam with no instructions and asking for me to take action. I don't have the phone number for CIC. Could someone please post it here or tell me where to find it. Thank you very much in advance.
