I'm not sure about the other things which you mentioned but I don't think having a USA visa affects the processing. And I did not raise a cse instead I was waiting for queries from the officer. Also, the loe was submitted at the time of aor. Please order gcms notes and check for analyst comments

If all the documents are in place and are verifiable then you don't need to worry, although it takes more than usual processing time. My AOR was feb 2018 and on march 9th there was a review comment was made by the analyst and and after that I directly got the ppr email on 17th september.

The comment from the Analyst is important to know as to why exactly he felt that the application should be reviewed. Your FSW points shows as 63 as per your comments but as far as I know the minimum entry requirement is 67, so please check what has bothered the analyst and if you think you can...

I was in review required for almost 7 months as it takes time when it goes in review. I'm not sure if there was any ghost update as my case was handled by an agent. I have rec'd my PPR now.

yes, but I had 3 different NOCs so if one wasn't accepted another two were there....however, If you have any docs you can submit them accordingly or better to wait for their reply.
