I think the medical clinic will automatically submit the results. He will receive an e-medical that you can submit along with the rest of the documents they requested.

Update: After 2 days of waiting for AOM, finally arrived in the Philippines. I'm in Canada in requested it online using PSA website.

Here's an update.. I registered our Marriage Certificate last May 3, 2016 and just received it back after yesterday. I had to call them and asked, seems like the City we applied at forgot to sent it back to us. Next step: Apply for AOM
