Finally got my SA letter dated 25th Apr 2016

Today morning, I received AOR1 ...application was received by CIC on 16th March, 2016

First of all congratulation for successful submission. You need not to worry for the signature, many people forget to check it. Since it is delivered to the correct address, the only option left is to wait for AOR1.

You can directly take an appointment and pay fees over there in Cash or Card(Debit or Credit). No forms are required for upfront medicals. Take passport during your medical examination and photograph as per direction from the Medical Centre while taking appointment. Take the original receipts...
