    No I don't think so. My first set of gcms notes was generated before the security check was complete and S15(1) was already in my notes.

    In fact, I just wrote my name and the Canadian address (CSIS did not specify that the applicant has to reside in Canada). No consent form was involved. I do know others attached the consent form and in both cases we all got the reply.

    Thank you! For the CSIS, yes I did ask my friend in Canada to fax it and leave a Canadian address. CSIS will mail a letter to the Canadian address. To do that you may have to have a person inside Canada to help:)

    I am an applicant living outside Canada, too. I just provided my name and my Canadian friend's address. Some outland applicants authorised their Canadian friends/relatives to request the information for them with a consent form. Either way, you may explore more information from the CSIS website.
