Okay, resolved! The problem was with my PDF viewer. Once I switched to Adobe everything worked perfectly, and the co-signer field was gone. Perfect. Hope this is helpful to someone else!

I managed to fix the phone number. Thanks! I wish I had asked sooner, this forum is amazing! But now when I have re-opened to document, it is 7 pages, and the co-signer page is there- I swear I am not making this up. Do I leave all of the co-signer fields totally blank? Thanks!
