Hi, I had this kind of situation before, that time I did nothing until they received my medical result and they sent me additional letter asking to update my current passport. Each case is different so it's better to call cic right away. Hope it will help you a bit

Well all I can say is more than 3 months before cic open up your file, I sent my extension last october and until now I did not hear anything yet. But since I got my permanent resident first I think it doesn not matter anymore even if they refused mine. Good luck mate
