Seems to be a similar situation like mine. Schedule A has been asked for my husband who is dependent applicant. And yes he has mentioned yes for 4th and 5th satutory questions. So as suggested by seniors, schedule A should be given as per the requirements ie dependent applicant in my case...

Yes . Send a signed letter stating that you require report in both names along with other documents. Also mention reference number on each copy.

Do send copy of passport( if it has new name) and copy of marriage certificate along with other documents. Also send a letter to them stating that you want the report in both names. ( Remember to write reference no on the letter and sign it.)
