    I photocopied every single page of all my US passports, (one with maiden name and new one with married name) that I held in the past 5 years, in black and white. Stamped and blank pages.     Just don't wait until the last minute to renew it. I would say send in the application 6 months before planning on moving back to Canada just to be on the safe side.
