i do not know the rush service charges but regular are $57 and 2 weeks. when you go to registry they will ask for your photo id and few questions about baby's birth, mothers name and all , no other form is needed.

Hello My baby was born on July 4th 2015 , Calgary and i went to registry to apply for Birth certificate on july 24th . Registry told me it may take upto 6 weeks but the birth certificate was delivered in 5 days. It takes about 3 weeks for them to update the system after the child is born...
