Ok thank you very much! So far we just have the medical finished for the child. What else would we need for him? I'm guessing just the passport and the non - accompanying parent form? What other forms from the dswd would we need if any?

Ok thanks for you help. Would I need to fill out a separate full application for the child or just fill out the additional dependants in my wife's app, check mark the accompanying to canada with her, and pay the additional fee ($150) for the child?
