    I doubt that too ! That the IMM 5669 is the same as Schedule A . Please help me if anyone knew the Appendix A form..... hellllllllpppppppppp...........

    hi sammystorm19 I opened the imm 5669 form but it says Schedule A instead of Appendix A. are they the same though ?

    I received an email from Manila Immigration and they told me to submit the following: -original passport of your dependent\s -2 pieces of passport-sized ID photos -Completed Appendix A(see attached) The problem is there's no Appendix A attachment. Is there anyone would help to find the Appendix...
