I have mailed to my VO as mentioned in the email from my VO so similarly you should mail to your respective VO. when VO received applicant's file they send email to applicant regarding receipt of his/her file. They send the medical and RPRF details as per my expreince

You have to send printout of receipt of payment please fill the receipt by hand name uci num etc etc. receipt is actually blank form that applicant have to fill before mailing to office. Also send proof of medical, Xerox of passport and one form attached to your email received from cic along...

Got email from NVDO that they have received my file and complete my case within 6 months. Till 6 months they asked me not to contact us due to high volume of files.

Got my LOA today 20 Sept 2014 in India (Punjab) and LOA date is 31 July 2014 Applied on 26 Nov 2013 Sponsor Interview was on 23 July 2014. :) :) :)
