Great... now the whole things are sorted out. Disclose the issue showing all the documents you have. And also a cover letter to explain how things happen and everything. Just that's it.

Better disclose, and try to get proofs that the old case and cancellation was a mistake.

Better you disclose that old issues and the reasons for cancellation in detail. So that they can understand that you didn't hide anything.
