I don't think so application can be returned as this is a relatively new change. Did you find any difference between the versions?

You still have to mention 2018 if you are submitting your application in Jan 2019 (last five years list). You have put yes for 'Required to fill' and put no for 'did not file'.

Yes of course if you are signing your application in 2018, you have to count taxes filed in the last five years i.e. 2013, 2014, 2015, 2016 and 2017. But if you sign your application in 2019, then 2018 will be considered. Thanks!

if you application was sent in January 2019, then your eligibility period should be Jan 2014 - Jan 2019, Isnt it?

It is last five years, and 2018 has to be added. That's my understanding. For January 2019 applications, years should be 2014, 2015, 2016, 2017, and 2018.
