I submitted my wife's passport today at VFS. They did not ask me any form/authorization letter, so I think it's not required. However, I would suggest to carry it on safer side.

I got my medicals done on 28th Feb. at Bangalore. As per the hospital, it was uploaded/submitted by 3rd March at their end. All the best mate.. you will get your 3rd line soon..!!!
