I am filling 5669E form for my spouse and I need additonal space for the form. In the additional form, should I write my spouse's name or what should I do? should I include my name(princial applicant) in the additional form. I am confused.

I also have the same question. What will happen if you are living abroad but travelling to different cities within the same country? I do not remember travelling to all cities exact dates.

What is an affidavit? My translator has a standard stamp which says translated is true to the original and where he puts date, signature and everything. Then I took that original to the notary where he has put another stamp saying true to the original and all his details.
