If your wife has no children,all you have to fill out is the principal applicant section and the rest should be NA You don't have to put your name in the spouse field as you are the sponsor

Thanks Chris, I don't have a uci yet since we just sent the application, We'll just wait to have our papers forwarded to Manila vo then we'll send the copy. The old one is just for proof though,since it was the passport he used in our honeymoon and overseas travels.
