Finally recieved PPR via email on (Saturday) May 28 and long wait of 5.5 year is over. Will submit Passport on Monday. If anyone has any questions please PM me.

You do not need to apply any extension. If you do not receive passport by due date then you are on "implied status". http://www.cic.gc.ca/english/resources/tools/temp/visa/validity/implied.asp

Yes it will be along with document request. Prepare all document beforehand to avoid delay.

You have to order CAIPS to find out what exactly is happening.

Hi MNZ, I know you are waiting for long time. I wonder why they are asking DNA test at this stage, they should have asked before. Do you know reason of delay ? From this forum, these people did DNA test in past : prince nijjar, Apply 2010 ,Lisa89, rajji53. You can try PM them, may be they help...

New Delhi does ask for documents not sure about your country but yes medical for sure.
